As part of Kahuku Medical Center's ongoing efforts to improve the accessibility and affordability of healthcare, patients in need of financial assistance can apply for the Community Care Program. Qualifying low-income, underinsured, or uninsured individuals may be eligible to receive healthcare services at a reduced cost or free of charge.
How Do I Apply?
Download the Community Care Program application found below. Completed applications must be submitted along with copies of all requested supporting documentation to determine program eligibility.
Note: All information submitted is subject to verification by Kahuku Medical Center. If any of the information provided is determined to be false, such a determination will result in the denial of any approved financial assistance, and the patient will become liable for all outstanding charges.
